Please arrive 15 minutes early for check-in. Bring your physical Insurance Card (front & back), a form of Identification, Co-Payment / Deductible payment method (cash, check, or card), a complete list of current medications and supplements, and any prior medical records if available. You can also download registration paperwork beforehand from our Forms section.
An annual wellness exam is a preventive service covered once per calendar year by most insurers to detect illness early and review risk factors. Note that if you have new or ongoing chronic illnesses evaluated during the same visit, those components and additional lab tests may be billed separately as standard medical care per insurance coding rules.
Bristol Primary Care LLC does not prescribe chronic benzodiazepines or narcotic pain medications. Our medical care emphasizes evidence-based, sustainable disease management and non-narcotic therapeutic options.
We allocate dedicated slots for sick visits within 24 to 48 hours, and same-day in-person appointments are available. Our physicians maintain a flexible schedule dedicating 30-45 minutes per patient so you receive thorough evaluation, on-site diagnostics, and prompt relief.
Our physicians hold hospital privileges across Waterbury Hospital, Hartford HealthCare, and Middlesex Hospital. If you are hospitalized, our physicians coordinate directly with the attending hospitalists and review all discharge summaries prior to your follow-up clinic visit.
